The ICAM Incident Investigation method is increasingly becoming preferred method for investigating incidents for organisations around the world and on many major projects and is being identified as a required skill for Workplace Health and Safety personnel. The ACT Safety ICAM Incident Investigation Lead Investigator Course has been developed around the Incident Cause Analysis Method which has been developed in association with Professor James T Reason in his research on Human Error and its contribution to incidents.
INCIDENT CAUSE ANALYSIS METHOD INVESTIGATION (ICAM Training) - Lead Investigation Techniques
NZQA 17601 - Produce an occupational health and safety incident investigation - Level 4, 6 Credits
It is now globally recognised as a peak incident investigation methodology and is widely used in industry for safety-related incidents. Many concepts such as the Swiss Cheese Model, Active and Latent Failures etc have now become part of the standard in the vocabulary of incident investigation. Numerous industries and many major construction and resources clients require their managers, team leaders, area superintendents, supervisors, safety officers, and safety representatives to be competent in using this robust and transparent methodology for incident investigations.

Our lead facilitators of this course have conducted many incident investigations for energy, mining, construction, and emergency services. We have been training this methodology for high risk industries such as extractives industry (mining and quarrying) for the past 10 years, including delivering this training for high hazard unit regulators and inspectors when they have been completing their units of competency, provide solid basis for either leading or contribute to these incident investigations.
